Individualized Therapeutic Activities
Offering specialized in-home therapeutic activities
based on fundamentals of dementia and Alzheimer’s
recreational guidelines. This is an excellent way
to enhance the daily living of persons living in
nursing homes, Independent Living or Assisted Living
with extra attention and one on one interaction.
Family members will receive monthly updates on
participation and level of functioning. A one time
assessment will be administered upon opening the
case to ascertain the appropriate type, session
length and level of activity needed. Residents love
the added attention and individualized activity that
goes over and above
what is offered in group activities offered at the
facility.

In-Home
Assessments
Assessment of the client's needs should be done in order to
create a care plan-here are the steps:
- Complete an individual assessment of the client's needs.
- Screen
the client for depression and capacity.
- Make
recommendations as needed (care planning)
- Assessments can be completed in either the private home
or facility where consumers reside. The person
responsible for the client must be in attendance during
all assessments.

Upon
completion of the assessment, a care plan with provider
listings will be provided the individual (or Power of
Attorney/Responsible Party) as indicated.

Consumer
Education, Family Support and Mediation Consults
Popular when caretakers/family members have differing ideas
on what should be done. After the initial assessment and
care planning, sometimes families (or caretaker groups) need
more explanations on why a particular care plan model may be
more advantageous over others. This also provides individual
caretakers with objective, neutral assistance in dealing
with other family members who may live out of the area and
who may not grasp the individual’s level of decline.
Longitudinal Studies
A system of standardized assessments, these can be done
quarterly to chart functional decline over time.
Longitudinal Studies are useful in providing caretakers with
an objective tool for decision making. These are also sent
to the individual’s Primary Care Physician (with permission)
to update doctors on pertinent functional information.

After
completing a Psychology degree at Southeastern Louisiana
University, Amy began providing care management services
to a variety of consumer groups, including Chronic Mentally
Ill Adults, At Risk Youth and Geriatrics in Baton Rouge and
the surrounding areas. Her career spans over 14 years and is
heavily concentrated in Geriatric Level of Care Assessments
and family consultations as mediator.
Amy grew up traveling from an early age, working with
medical missionary groups to create start-up privately
sponsored medical services and clinics in remote areas of Mexico and Central America. Today, she continues to work
in the supply side of moving donated medical equipment and
approved medications to Christian Medical Groups traveling
out of the country.
Locally, she is working as the Victim’s Assistance
Coordinator for the Diocese of Baton Rouge under the Charter for the Protection of Children and Young People.
Periodically, she writes articles about women’s issues and
issues facing the elderly, which are posted in Catholic Healthcare Publications and Geriatric
Association Publications. Amy is familiar with the
various levels of care offered in the community and which
programs are best suited to meet a client’s specific needs.
She is the author of Girl In My Closet and another
upcoming book named Girl Relics.
Amy is bilingual and can provide services in English and
Spanish. She is a member of St. George Catholic Church in
Baton Rouge, Louisiana and works full time with one of Baton
Rouge’s leading Home Health Care agencies providing
community teaching and writing for its monthly healthcare
newsletter.
